Claiming hospital treatment

If your claim relates to treatment in hospital, your claim will be automatically completed for your signature and forwarded to CUA Health. You need only pay any out of pocket costs that may be applicable to the product you have selected.

Claiming extras services

When using your cover under extras benefits, you may use your CUA Health policy holder card for on-the-spot claims at participating HICAPS (health insurance claims and payments service) or iSoft (IBA Health Group Limited) registered provider. 

Alternatively you can simply complete a claim form, attach both the bill and receipt, then either post it to CUA Health, or forward through your local branch.
