Pre-existing medical condition declaration forms
If you don't have any pre-existing medical conditions and you're under 81 years of age simply obtain an insurance quote with the option to then purchase your travel insurance online.
Please be aware of the definition of pre-existing medical conditions. If you have any pre-existing medical conditions please complete one of the following:
- pre-existing medical declaration form for under 81
- pre-existing medical declaration form for over 81's
Fax the complete form directly to Mondial Assistance on 07 3305 7006.
Once you have received confirmation from Mondial Assistance, obtain an insurance quote and read the different types of cover available, then purchase your travel insurance.
